When preparing for labor, most parents spend countless hours learning about contractions, cervical dilation, and pushing. But there’s one important stage of birth that often receives very little attention—the third stage of labor.
Even though your baby has already been born, your body still has one final task to complete: delivering the placenta.
This stage may only last a few minutes, but it plays a significant role in postpartum recovery, preventing excessive bleeding, and even establishing breastfeeding. Understanding what happens during the third stage of labor can help you feel more informed and confident when discussing your birth preferences with your healthcare provider.

What Is the Third Stage of Labor?
Labor is divided into three stages:
- First Stage: Labor contractions and cervical dilation.
- Second Stage: Pushing and the birth of your baby.
- Third Stage: Delivery of the placenta.
The third stage begins immediately after your baby is born and ends once your placenta has been delivered.
For most births, this stage is surprisingly short. The placenta is typically born within 2–15 minutes, although a normal third stage can sometimes last up to 30 minutes depending on the circumstances.
Although parents often shift their attention completely to their newborn (understandably!), your healthcare team is closely monitoring you during this time because the delivery of the placenta has a direct impact on postpartum bleeding.
Why Is the Third Stage So Important?
Throughout pregnancy, the placenta acts as your baby’s life-support system.
It provides oxygen and nutrients while removing waste products through an extensive network of blood vessels connecting the placenta to your uterus.
After your baby is born, those blood vessels remain open until the placenta separates and your uterus contracts firmly enough to compress them.
This process is important because it helps prevent postpartum hemorrhage, one of the leading causes of serious complications after birth.
Complete placental delivery is also essential for normal postpartum hormone changes that stimulate mature milk production.
What Happens Inside Your Body After Birth?
Immediately after birth, your body experiences another surge of oxytocin, often called the “love hormone.”
Holding your baby skin-to-skin, making eye contact, smelling your newborn, and beginning breastfeeding all naturally increase oxytocin levels.
Those oxytocin surges continue causing your uterus to contract.
While you’re soaking in those first moments with your baby, your uterus is working behind the scenes to:
- Separate the placenta from the uterine wall
- Compress the blood vessels where the placenta was attached
- Minimize blood loss
- Push the placenta into the birth canal
Many parents don’t even notice these postpartum contractions because they’re focused on meeting their baby.
Signs the Placenta Has Separated
Healthcare providers look for several signs that indicate the placenta has naturally detached and is ready to be delivered.
These include:
- A sudden gush of blood
- Lengthening of the umbilical cord
- A change in the shape or position of the uterus
- Stronger cramping
- Feeling another urge to push
These signs suggest that the placenta has separated and is ready to be born.
Two Approaches to Delivering the Placenta
There are two accepted ways to manage the third stage of labor.
Physiologic (Expectant) Management
Physiologic management allows the body to complete the third stage naturally without routine medications.
Your provider simply waits for the placenta to separate on its own while monitoring bleeding and watching for the normal signs of placental separation.
This approach often includes:
- Immediate skin-to-skin contact
- Delayed cord clamping (when appropriate)
- Waiting for spontaneous placental separation
- Pushing only when your body signals it’s time
Many families planning low-intervention or physiologic births choose this option because it supports the body’s natural hormonal processes.
Active Management
Active management is the standard approach in many hospitals and is primarily intended to reduce the risk of postpartum hemorrhage.
This typically includes:
- Pitocin (synthetic oxytocin) given by IV or injection
- Gentle controlled cord traction after placental separation
- Ongoing monitoring of bleeding and uterine tone
Pitocin helps the uterus contract more effectively, encouraging placental separation while also compressing the blood vessels to reduce bleeding.
Research has consistently shown that active management lowers the overall risk of postpartum hemorrhage, particularly for individuals with known risk factors.
Which Option Is Right for You?
Neither approach is automatically better.
The safest option depends on:
- Your pregnancy history
- Your labor experience
- Your medical conditions
- Your personal preferences
- Your current bleeding and overall health after birth
For someone with a completely uncomplicated birth and no risk factors, physiologic management may be appropriate.
Someone with previous postpartum hemorrhage or anemia may benefit from active management.
Birth is individualized—and so is third-stage management.
Risk Factors for Postpartum Hemorrhage
Healthcare providers continuously assess your bleeding risk throughout pregnancy, labor, and after birth.
During Pregnancy
Risk factors include:
- Previous postpartum hemorrhage
- Anemia
- Gestational diabetes
- Gestational hypertension or preeclampsia
- Carrying twins or multiples
- Large baby
- Excess amniotic fluid
- Blood clotting disorders
During Labor
Additional risks include:
- Labor induction
- Labor augmentation with Pitocin
- Very rapid labor
- Very prolonged labor
- Infection
- Operative vaginal birth (vacuum or forceps)
- Elevated blood pressure
After Birth
Providers continue monitoring for:
- Excessive bleeding
- Poor uterine tone
- Delayed placental delivery
- Changes in blood pressure or heart rate
These factors help guide recommendations for third-stage management.
What Is a Retained Placenta?
A retained placenta generally means the placenta has not been delivered within approximately 30 minutes after birth.
However, time alone isn’t the only consideration.
Providers also assess:
- Amount of blood loss
- Maternal vital signs
- Uterine firmness
- Signs that the placenta is separating
Some people can safely wait longer if bleeding remains minimal, while others require earlier intervention if blood loss becomes significant.
What Happens If the Placenta Doesn't Deliver?
If the placenta remains attached, providers usually start with conservative interventions before moving to more invasive treatments.
These may include:
Breastfeeding
Nursing stimulates natural oxytocin release, encouraging stronger uterine contractions.
Changing Positions
Standing, squatting, or walking may use gravity to help the placenta separate.
Additional Pitocin
Medication may be given to strengthen uterine contractions if needed.
If bleeding becomes excessive or the placenta remains firmly attached, more advanced interventions may be necessary.
Manual Removal of the Placenta
If the placenta still cannot be delivered safely, your provider may recommend manual removal.
This involves carefully inserting a hand into the uterus to separate and remove the placenta.
Occasionally, a dilation and curettage (D&C) procedure is needed if retained tissue remains.
Because these procedures increase infection risk, antibiotics are often recommended afterward.
Although manual removal is uncomfortable and more invasive, it can be a lifesaving intervention when heavy bleeding occurs.
Retained Placental Fragments
Sometimes the placenta appears complete but small pieces remain inside the uterus.
Retained placental tissue can lead to:
- Heavy postpartum bleeding
- Delayed postpartum hemorrhage
- Uterine infection
- Delayed milk production
- Prolonged postpartum recovery
Complete placental delivery is essential because it triggers the hormonal shift that tells your body to begin producing mature breast milk.
Placenta Accreta: When the Placenta Is Too Firmly Attached
If the placenta does not separate normally, providers may evaluate for placenta accreta spectrum.
This occurs when the placenta grows too deeply into the uterine wall.
Risk factors include:
- Previous Cesarean birth
- Prior uterine surgery
- Previous placenta previa
- Other uterine procedures
Placenta accreta requires specialized management and may involve surgical treatment.
The Bottom Line
The third stage of labor may only last a few minutes, but it has a significant impact on your postpartum recovery.
Understanding how the placenta separates, recognizing the signs of normal placental delivery, and knowing the differences between physiologic and active management can help you make informed decisions during birth.
