If you’ve reached the end of your pregnancy, chances are you’ve heard your provider mention the possibility of scheduling an elective induction. Maybe they’ve recommended an induction at 39 weeks, or perhaps they’ve already suggested putting a date on the calendar.
If you’re wondering whether an elective induction is the right choice for you, you’re not alone.
The decision to induce labor is personal, and while research has changed how providers discuss elective induction, there isn’t a one-size-fits-all answer. Understanding the evidence, the potential benefits, the possible risks, and—most importantly—your own birth preferences can help you make an informed decision.

An elective induction is the process of medically starting labor without a medical reason between 39 and 41 weeks of pregnancy.
Unlike medically indicated inductions—which may be recommended for conditions like:
- Gestational diabetes
- Preeclampsia
- High blood pressure
- Poor fetal growth
- Cholestasis
- Ruptured membranes without labor
an elective induction is offered simply because you’ve reached full term and are still pregnant.
It’s important to remember that an elective induction is exactly that—elective.
Even if your provider recommends scheduling one, you still have the right to ask questions, understand the risks and benefits, and decide whether or not it’s the best choice for you.
Your pregnancy. Your body. Your decision.
Why Are Elective Inductions Offered Starting at 39 Weeks?
Many parents wonder why providers don’t offer elective inductions earlier—or wait until after 41 weeks.
The answer comes down to balancing the health of both the baby and the birthing parent.
Baby’s Lungs Are Fully Developed
By 39 weeks, most babies have completed lung development, significantly reducing the chance they’ll need breathing support after birth.
While babies born at 37 weeks are considered “early term,” they are still more likely to experience:
- Respiratory distress
- NICU admission
- Feeding challenges
- Difficulty transitioning outside the womb
Waiting until 39 weeks allows babies additional time to mature while still minimizing risks associated with prolonged pregnancy.
The Risk of Stillbirth Changes Over Time
Although stillbirth remains rare, research shows that the risk gradually increases as pregnancy continues.
Approximate risk of stillbirth by gestational age:
- 37 weeks: 1 in 10,000
- 38 weeks: 1.5 in 10,000
- 39 weeks: 4 in 10,000
- 40 weeks: 7 in 10,000
- 41 weeks: 17 in 10,000
- 42 weeks: 32 in 10,000
These numbers help explain why many providers begin discussing induction around 39 weeks while recommending delivery before or around 42 weeks.
It’s important to recognize that while the risk increases over time, it remains relatively low overall.
The ARRIVE Trial: Why Everyone Started Talking About Elective Induction
Much of today’s conversation surrounding elective induction stems from the ARRIVE Trial, a large randomized controlled study published in 2018.
Researchers compared two groups of first-time parents with uncomplicated pregnancies:
- Elective induction at 39 weeks
- Expectant management (waiting for labor to begin naturally)
To participate, pregnancies had to meet strict criteria:
- First pregnancy
- One baby
- Baby in a head-down position
- Low-risk pregnancy
- No medical complications
Because participants were randomly assigned to each group, the study provided high-quality evidence regarding outcomes.
What Did the ARRIVE Trial Find?
The results surprised many providers.
Lower Cesarean Birth Rate
One of the biggest findings was a slightly lower cesarean rate among those who had elective inductions.
- Elective induction: 19%
- Expectant management: 22%
Historically, many believed induction automatically increased cesarean risk.
This study challenged that assumption.
Similar Outcomes for Babies
Researchers found very few differences in newborn outcomes.
Babies in both groups generally did well.
The induction group showed:
- Slightly less need for prolonged respiratory support
- Slightly shorter newborn hospital stays
However, many experts believe these findings may be partly explained by the higher cesarean rate in the expectant management group, since cesarean births can increase respiratory challenges for newborns.

Only about 6% of births in the study were attended by midwives.

Many hospitals in the United States have cesarean rates between 30–35%, while the study reported rates closer to 20%.

Rather than asking, “Should I be induced?”
Consider asking yourself these questions instead.
What Kind of Birth Do I Want?
If your goal is:
- Spontaneous labor
- Minimal medical intervention
- Unmedicated birth
then elective induction may not align with those priorities.
If your goal is:
- Planning childcare
- Reducing uncertainty
- Scheduling around work or family
- Avoiding pregnancy beyond 40–41 weeks
then induction may feel like the better fit.
What Is My Provider’s Cesarean Rate?
Ask your provider:
- What is your induction cesarean rate?
- How often do first-time parents undergoing induction have vaginal births?
- How long do you typically allow an induction to continue before recommending a cesarean?
These answers often tell you more than national statistics.
Where Am I Planning to Give Birth?
Hospital births commonly offer elective induction.
Birth centers and home birth settings generally do not.
Choosing an elective induction often means transferring care to a hospital if you’re planning an out-of-hospital birth.
Are My Due Dates Accurate?
Accurate dating matters.
If your due date is incorrect, an induction thought to be happening at 39 weeks could actually occur much earlier, increasing risks for your baby.
Benefits of Elective Induction
For many families, elective induction offers meaningful advantages.
Potential benefits include:
- Lower risk of developing pregnancy-related hypertension
- Slightly lower cesarean rate in some research
- Reduced risk of stillbirth associated with prolonged pregnancy
- Ability to schedule childcare and family support
- Predictability around provider availability
- Relief from the physical discomforts of late pregnancy
- Potentially shorter postpartum hospital stay
Many families appreciate having a plan, especially when coordinating childcare for older siblings or family travel.
Potential Risks of Elective Induction
Like any medical intervention, elective induction also has potential drawbacks.
Possible risks include:
Longer Labor
Inductions often take longer than spontaneous labor.
Depending on cervical readiness, labor may last:
- 12–24 hours
- 24–48 hours
- Occasionally several days
More Medical Interventions
Induction often involves multiple procedures, including:
- Cervical ripening medications
- Balloon catheter
- Pitocin
- Artificial rupture of membranes
- Continuous fetal monitoring
Each intervention may influence the course of labor.
Increased Use of Pain Medication
Research consistently shows people undergoing induction are more likely to use epidurals or other pain medications.
This doesn’t mean an unmedicated induction is impossible—but it may be more challenging.

- Can I still move, eat, and use comfort measures during labor?
Open conversations help you make informed decisions rather than feeling like choices are made for you.
